Com a aprovação pela Agência Nacional de Vigilância Sanitária (Anvisa), no final de março, do registro do medicamento sofosbuvir, está próxima a disponibilização de um tratamento mais eficaz e rápido contra a hepatite C, no Brasil. De acordo com o diretor do Departamento de DST, Aids e Hepatites Virais, do Ministério da Saúde, Dr. Fábio Mesquita , o tratamento completo – que engloba mais outros dois remédios – deve ser disponibilizado pelo Sistema Único de Saúde (SUS) até julho deste ano.

“Após o registro pela Anvisa, agora só falta a aprovação do sofosbuvir pela Comissão de Incorporação Tecnológica do SUS (Comitec), e isso deve ocorrer entre abril e maio. Temos a expectativa que os medicamentos estarão disponíveis pelo SUS em junho, no máximo em julho”, afirma o diretor.

Em 2015, a Agência já havia aprovado o registro do daclatasvir – em janeiro –, e do simeprevir – em março. O Brasil será um dos primeiros países a adotar as novas tecnologias na rede pública de saúde.

O sofosbuvir, que tem o nome comercial Sovaldi, é de via oral e, juntamente com os outros dois medicamentos, pode curar eficazmente a hepatite C em 90% dos pacientes e evitar efeitos colaterais como cansaço e dores articulares. Além disso, pode dispensar o uso de injeções de interferon, beneficiando principalmente os pacientes que não toleram essa medicação e reduzindo de um ano para três meses o tempo necessário de tratamento.

Questionado se há possibilidade de medicamentos genéricos, o Dr. Fábio explica que empresas da Índia, China e até do Brasil poderão vir a produzi-los futuramente. No entanto, como esses produtos são recentes, ainda não há iniciativas nesse sentido.

Os produtos são tão novos que ainda estão em processo de análise de patentes. “Mas como são muito eficientes e negiciamos um excelente preço, não conseguimos esperar até saírem as patentes. Então, vamos colocá-los no SUS e, mais para frente, vemos o que vai acontecer em relação a isso”, esclarece o diretor do Ministério da Saúde.

Dr. Fábio explica que quem já está em tratamento para a hepatite C não deverá interrompê-lo. “O combinado no protocolo e o que é mais seguro de se fazer é que pacientes vão até o final do tratamento atual e, se houver falha, aí sim incluimos nesse método novo. Isso porque 50% deles vão se beneficiar do tratamento atual, ajudando, posteriormente, a cuidar dos outros 50% que não tiverem êxito”, aponta.

A hepatite C é causada pelo vírus C (HCV). A transmissão ocorre, dentre outras formas, por meio de transfusão de sangue, compartilhamento de material para preparo e uso de drogas, objetos de higiene pessoal – como lâminas de barbear e alicates de unha –, além de outros objetos contaminados com o vírus utilizados na confecção de tatuagem e colocação de piercings. Há também transmissão vertical (de mãe para filho) e sexual. Como a doença é silenciosa e apresenta sintomas em fases avançadas, muitos dos infectados desconhecem o diagnóstico.

Medicamento preventivo para HIV

O Brasil também deve se tornar pioneiro também ao combate ao HIV. Caso sejam bem-sucedidos dois estudos em andamento que avaliam a aceitabilidade da população brasileira ao remédio de consumo diário chamado Truvada, que serve para a prevenção ao vírus, o medicamento poderá ser disponibilizado pelo SUS em 2016.

A estratégia – conhecida como PrEP – é uma iniciativa adotada atualmente nos Estados Unidos, mas o medicamento só é disponibilizado pelas empresas seguradoras de saúde, não pelo governo. “O Brasil pode se tornar o primeiro país do mundo a oferecer medicamento de PrEP pela rede de saúde pública”, destaca o Dr. Fábio. Ainda de acordo com ele, a expectativa é que as pesquisas no País sejam concluídas ainda este ano.

Apesar de ser preventivo, especialistas alertam que o PrEP não é um substituto da camisinha, mas uma alternativa a quem não consegue usar preservativo. Por aqui, os estudos são conduzidos pelo PrEP Brasil. O objetivo é analisar o uso de Truvada por 500 homens que fazem sexo com homens (HSH), travestis e mulheres transexuais com risco de adquirir a infecção pelo HIV. Cada participante será avaliado no período de um ano por uma equipe especializada de médicos, enfermeiros e psicólogos durante todas as visitas do estudo. Ao todo, cerca de 100 profissionais estão envolvidos em dois locais em São Paulo (USP e CRT DST-Aids) e um no Rio de Janeiro (Fiocruz).

Since the registry of the sofosbuvir was approved by the National Sanitary Surveillance Agency (Anvisa) by the end of March, a new faster and more effective treatment is each time closer in Brazil.  According to the director of the STD, AIDS and Viral Hepatitis department from the Health Ministry, Dr. Fábio Mesquita, the complete treatment – which includes two other drugs – should be available by the Unified Health System (SUS) until July 2015.

After Anvisa released the registry, the sofosbuvir still need approval from the SUS Technology Incorporation Commission (Comitec), and this must happen between April and May. We expect these drugs to be available through the SUS in June, at most in July at latest, said the director.

In 2015, the Agency had already approved the daclatasvir – in January – and the simeprevir – in March. Brazil will be one of the first countries to use new technologies in the public health system.

The sofosbuvir, also known as Sovaldi, is administered orally and associated to two other drugs, can effectively cure hepatitis C in 90% of the cases and avoid side effects as fatigue and joint aches. Besides this, it can dismiss the use of interferon injections, a benefit for those who do not tolerate this drug and decreasing the treatment time from 1 year to 3 months.

Questioned if there is a possibility of generic drugs, Dr. Fábio explains that Indian, Chinese and even Brazilian companies will produce the drug in the future. However, since these products are new, there are no initiatives in this sense.

The products are so new that they are still in patent analysis process. But since they are very efficient and we have negotiated an excellent price, we could not wait until the patents were ready. This way, we will put them in the SUS and, further on, evaluate what happens, clarifies the director at the Health Ministry.

Dr. Fábio explains that those already being treated for hepatitis C must not interrupt their treatments. The protocol states what is safer, which means these patients will finish their current treatments, and if it fails, then we will include them in the new method. This happens because 50% of them will benefit from the current treatment, helping subsequently to care for the other 50% who were not successful, points.

When the treatment is released by the SUS, the goal is to attend from 60 to 90 thousand patients in two years. In Brazil, the Health Ministry estimates from 1.4 to 1.7 million people are currently infected with the virus, being most of them above 45 years of age.

The C virus (HCV) causes hepatitis C. The transmission occurs, between other ways, through blood transfusions, sharing of drug use or preparation materials, personal hygiene objects – as razor blades and pliers – besides other infected objects used in tattooing or piercing. The transmission can also be vertical (from mother to child) and sexual. Since the disease is silent and only develops symptoms in advanced stages, many infected people are unaware of their condition.

HIV preventive drug

Brazil should also become pioneer fighting HIV. If there is success in two studies in course, which evaluate the acceptance of the Brazilian population towards the daily use of a drug known as Truvada, which acts to prevent against the HIV, the drug may be available in the public health system in 2016.

The strategy – known as PrEP – is an initiative currently in use in the USA, but the drug is only available from the health insurance companies, not by the government. Brazil could be the first country in the world to offer PrEP drugs in the public health system, stresses Dr. Fábio. Still according to him, the expectation is that researches in the Country should be concluded this year.

Despite being preventive, experts alert that PrEP is not a substitute for condoms, but an alternative for those who cannot use them. Here, studies are conducted by the PrEP – Brazil the goal is to analyze the use of Truvada among 500 men who have sex with men (MSM), transvestites and transsexual women with risk of being infected by the HIV. Each participant will be evaluated during a year by a team of expert doctors, nurses and psychologists during all visits of the study. At all, around 100 professionals are involved in two sites in Sao Paulo (USP and CRT DST-AIDS) and one in Rio de Janeiro (Fiocruz).
